Information on this site is general and is not a guarantee of eligibility, pricing, benefits, coverage or acceptance. Coverage, carriers and plan options vary by state and individual circumstances. No coverage is effective until confirmed by the applicable carrier.

Consultation and enrollment assistance

Consultation and enrollment assistance are provided at no cost to the consumer. Insurance premiums, deductibles, copays and other plan costs are separate. There is no obligation to enroll. This website is a solicitation for insurance.

SMS program description

When you provide a U.S. mobile number and affirmatively consent, Unity Shield Family Insurance may send automated text messages for one-time phone verification, appointment scheduling, appointment confirmations, reminders and related service communications. Message frequency varies according to your activity and scheduled appointments.

SMS consent, charges and opt-out

Consent to receive text messages is not a condition of purchasing any product or service. Message and data rates may apply. Carriers are not liable for delayed or undelivered messages. Reply STOP to cancel future text messages. Reply HELP for help, call 727-503-6771, or email ashrithahealth@gmail.com. After an opt-out request, you may receive one final message confirming the opt-out.

Acceptable use

You agree to provide accurate information, use the appointment system only for genuine inquiries, and not interfere with the website, verification service or scheduling system. We may limit access when we reasonably suspect fraud, abuse or unlawful activity.

Limitation and changes

To the extent permitted by law, the website is provided as available without warranties regarding uninterrupted operation. We may update these terms and will post the effective date above. Continued use after an update constitutes acceptance of the revised terms.
